Isolierte Ventrikelseptumruptur nach stumpfem Thoraxtrauma

Zusammenfassung

An Hand einer eigenen Beobachtung werden Unfallmechanismus, klinische Symptomatik und Therapie des traumatischen Ventrikelseptumdefektes diskutiert. Mitteilenswert erscheint die offenbar noch nicht beschriebene isolierte Ruptur des Septum membranaceum mit Beteiligung der Scheidewand zwischen linkem Ventrikel und rechtem Vorhof. Die Ursache der isolierten traumatischen Ventrikelseptumruptur wird durch die Entwicklungsgeschichte des Herzens zu deuten versucht. Trotz ungewöhnlich rascher Diagnosestellung und operativer Korrektur verstarb der Patient am 11. postoperativen Tag, da die ungünstige präoperative Hämodynamik mit Schocksymptomatik zu Sekundärkomplikationen führte.

Summary

The mechanism, symptoms and treatment of traumatic ventricular septal defect are discussed on the basis of an own observation. To the best of our knowledge this is the first report of a rupture of the membranous septum, involving the portion between the left ventricle and the right atrium. An attempt is made to explain the mechanism of isolated traumatic ventricular septal rupture on the basis of embryological considerations. The patient died on the 11th postoperative circulatory status and inspite rather prompt diagnosis and operative treatment.
