Zusammenfassung
Kardiochirurgische Eingriffe mit extrakorporaler Zirkulation sind mit einem erhöhten Infektionsrisiko verknüpft. Immunbiologische Untersuchungen zeigten, daß postoperativ ein passagerer Abfall aller Immunglobulinspiegel auftritt. Hinsichtlich der zellulären Immunität fand sich eine deutliche Verminderung der Lymphozytentransformierbarkeit in Gegenwart von polyklonalen Mitogenen, die sich nach 8 Tagen nur teilweise normalisiert hat. Außerdem kommt es am 5. postoperativen Tag zu einer hohen Spontantransformationsrate der Lymphozyten. Diese Befunde sprechen für eine komplexe Alteration im immunologischen Apparat, die phasenhaft als Immundefizienz imponiert und zur Erklärung eines erhöhten Infektionsrisikos beitragen kann.
Summary
Cardiovascular Operations are connected with an increased risk of infection. Postoperatively there happened a temporary fall of all immunglobulin levels. Concerning the cellular immunity, we found an impressive decrease of the transformation of the lymphocytes in the presence of polyclonal mitogens which normalises only partially after the eighth day. These results refer to a complex alteration of the immunological apparatus, imposing as a temporary immundeficiency, and could contribute to an elucidation of the increased risk of infections.
