Herznahe Obstruktionen der unteren Hohlvene: Morphologische Besonderheiten und chirurgisches Vorgehen

Zusammenfassung

Operable Obstruktionen des herznahen Anteils der unteren Hohlvene (VCI) sind insgesamt selten, obwohl sie neuerdings häufiger erkannt werden. Anhand von 2 Patienten, die wir seit 1969 wegen einer Budd-Chiari-Symptomatik operierten, werden Diagnostik, Morphologie und operative Möglichkeiten der membranösen Obstruktionen dargestellt. Im Fall einer Patientin, die zweimal operiert wurde, fanden wir die bisher in der Literatur nicht beschriebene Kombination einer infrakardialen Membranstenose und einer großen, die VCI obstruierenden Valvula Eustachii.

Summary

Operable obstructions of the hepatic portion of the inferior vena cava (IVC) are rare but probably diagnosed more frequently nowadays. Since 1969 we operated 2 patients, one of them twice. Concerning to these cases the problems of diagnosis, morphology and surgical treatment are discussed. In the patient, who was operated twice, we found the combination – which has not been reported in medical literature yet – of a membranous stenosis of the hepatic portion of the IVC and a large Eustachian valve obstructing the IVC.
