Zusammenfassung
Halbwirbel sind die häufigste kongenitale Fehlbildung der Wirbelsäule und damit gleichzeitig die häufigste Ursache kongenitaler Wirbelsäulendeformitäten. Da sie durch Fehler der Formation im Sinne unvollständig ausgebildeter Wirbelkörper früh in der Embryogenese entstehen, zählen sie zu den Formationsstörungen. Je nach Alter, Grad der Segmentation des Halbwirbels und Lokalisation haben sie ein unterschiedliches Risiko für die Ausbildung von Deformitäten. Einer konservativen Therapie sind sie kaum zugänglich. Operative Korrekturen haben das Ziel der Entwicklung einer balancierten Wirbelsäule in der koronaren Ebene mit einem regelrechten sagittalen Profil. Operative Interventionen sollen so kurzstreckig wie nur irgend möglich gehalten werden. Diese Ziele lassen sich gut erreichen, wenn die Fehlbildungen früh erkannt und möglichst bereits vor dem präpubertärem Wachstumsschub operativ korrigiert werden.
Abstract
Hemivertebrae are the most common congenital spinal malformation and the most common cause of congenital spinal deformities. They are the result of formation defects in the sense of incompletely formed vertebral bodies early in embryogenesis. Depending on age, degree of segmentation and localization of the hemivertebra, they have different risks of developing deformities. They are hardly amenable to conservative treatment. The aim of surgical correction is to achieve both a balanced spine in the coronal plane and a correct sagittal profile. Surgical corrections should be kept as short as possible. The earlier these deformities are recognized and surgically corrected, the better these goals can be achieved.
