Die aktuellen Leitlinien zur Pharmakotherapie der Herzinsuffizienz mit reduzierter Pumpfunktion empfehlen den frühzeitigen und möglichst simultanen Beginn mit einem Betablocker (BB), SGLT2-Inhibitor (SGLT2i), Mineralokortikoidrezeptor-Antagonisten (MRA) und ACE-Hemmer (ACEi) bzw. alternativ einem Angiotensin-Rezeptor-Neprilysin-Inhibitor (ARNI). Dabei wird die Sequenz des Therapiestarts der empfohlenen Wirkstoffklassen und deren Erweiterung und Dosissteigerung im Verlauf unter Berücksichtigung der individuellen Patientenmerkmale und Komorbiditäten bewusst dem behandelnden Arzt überlassen. Somit wird das bisherige Stufenschema mit einer sequenziellen Initiierung der verschiedenen Wirkstoffklassen verlassen, da diese primär auf der Historie der zugrunde liegenden klinischen Endpunktstudien basieren und nicht den frühzeitigen und synergistischen prognoseverbessernden Effekt der Wirkstoffklassen berücksichtigen. Die Empfehlung zur Dosissteigerung auf die in den klinischen Studien verwendeten Zieldosen von BB, SGLT2i, MRA und ACEi bzw. ARNI bleibt aber erhalten. Die Erweiterung um weitere Wirkstoffklassen sollte in Abhängigkeit von Komorbiditäten, klinischen Parametern, Progress der Herzinsuffizienz und Evidenzlage aus klinischen Studien erfolgen.
Abstract
The current guidelines for the pharmacotherapy of heart failure with reduced ejection fraction recommend the early and, if possible, simultaneous start of a beta-blocker (BB), SGLT2-inhibitor (SGLT2i), mineralocorticoid receptor antagonist (MRA) and ACE inhibitor (ACEi) or, alternatively, angiotensin receptor neprilysin inhibitor (ARNI). The choice of these drug classes used at the beginning and their expansion and dose increase over the course of time is deliberately left to the treating physician, taking into account the individual patient characteristics and comorbidities. This means that the previous recommendations for a sequential initiation of the various drug classes are abandoned, as these are primarily based on the history of the underlying clinical endpoint trials and do not do justice to the early and synergistic prognosis-improving effects of the drug classes. However, the recommendation to increase the dose of BB, SGLT2i, MRA and ACEi/ARNI to the target doses used in clinical trials remains in place. The inclusion of other drug groups should depend on comorbidities, clinical parameters, progression of heart failure and evidence from clinical studies.
Der frühzeitige und möglichst simultane Beginn mit einem Betablocker (BB), SGLT2-Inhibitor (SGLT2i), Mineralokortikoidrezeptor-Antagonisten (MRA) und ACE-Hemmer (ACEi) bzw. alternativ einem Angiotensin-Rezeptor-Neprilysin-Inhibitor (ARNI) ist die zentrale Änderung in den neuen Empfehlungen zur Pharmakotherapie der Herzinsuffizienz mit reduzierter Ejektionsfraktion und soll den frühzeitigen und synergistischen prognoseverbessernden Effekt der Wirkstoffklassen bei der Behandlung sicherstellen. Die Wahl der zu Beginn verwendeten Wirkstoffklassen und deren Erweiterung und Dosissteigerung im Verlauf unter Berücksichtigung der individuellen Patientenmerkmale und Komorbiditäten wird dabei bewusst dem behandelnden Arzt überlassen.
