Pediatric Lens Subluxation Surgery, Aphakic Glaucoma, Hemangioma and Dissociative Visual Loss

The management of congenital or childhood subluxation of the crystalline lens is difficult. With increasing subluxation, an irregular astigmatism arises that may not be sufficiently corrected with glasses and results in childhood amblyopia. Lens surgery is associated with the loss of accommodation and the risk of postoperative complications including glaucoma and retinal detachment. However, postponing the surgery increases the surgical difficulty. Van Os et al. [1] present a new surgical technique based on a combination of bean-shaped segments to support the bag-in-the-lens and a 6 – 0 polypropylene loop fixated at the sclera. They describe the results of seven eyes of four cases of children with crystalline lens subluxation and an astigmatism of 6.5 diopters or higher, which could be reduced into a range of 0.25 to 3 while achieving a stable lens fixation. They provide details of the surgery including postoperative treatment.
