Zusammenfassung

Die Anwendung von Botulinumtoxin (BTX) ist inzwischen der Goldstandard zur Behandlung von Lidkrämpfen (essenzieller Blepharospasmus, Hemispasmus facialis, Synkinesien nach Fazialisparese). Weitere wichtige Indikationen sind die Behandlung von Lidretraktionen bei endokriner Orbitopathie und die Induktion einer protektiven Ptosis bei Lagophthalmus oder schweren Hornhauterkrankungen.

Abstract

Botulinum toxin has been first applied in ophthalmology (as treatment for strabismus) and is nowadays widely used in many clinical areas. Currently the main application in ophthalmology is the treatment of essential blepharospasm and hemifacial spasm. Other important indications for botulinum toxin in ophthalmology are eyelid retraction in Gravesʼ disease and the induction of protective ptosis in lagophthalmos or corneal diseases.

  • Regelmäßige Injektionen mit Botulinumtoxin sind der Goldstandard zur Behandlung des essenziellen Blepharospasmus und des Hemispasmus facialis. Hier besteht eine zugelassene Indikation.

    • Die Diagnose ist klinisch. Beim Hemispasmus facialis sollte initial ein MRT durchgeführt werden.

    • Die Botulinumtoxin-Injektionen werden in den M. orbicularis oculi gegeben.

    • Jeder Patient benötigt ein individuelles Injektionsschema.

    • Die Wirkdauer beträgt beim essenziellen Blepharospasmus in der Regel 2–3 Monate, beim Hemispasmus facialis meist 3 oder mehr Monate.

  • Periorbitale Injektionen sind auch bei der Fehlregeneration nach Fazialisparese sinnvoll.

  • Weitere wichtige Indikationen in der Augenheilkunde/im Augenbereich:

    • Induktion einer protektiven Ptosis bei Lagophthalmus und schweren Hornhauterkrankungen,

    • Behandlung der Lidretraktion bei der endokrinen Orbitopathie,

    • Injektionen in Augenmuskeln bei Abduzensparalyse und Vertikaldeviationen bei aktiver endokriner Orbitopathie.

  • Anzahl und Ausmaß der Nebenwirkungen sind nicht von der Therapiedauer abhängig. Sie umfassen:

    • Hämatome als häufigste Nebenwirkungen beim essenziellen Blepharospasmus und Hemispasmus facialis,

    • leichte Ptosis,

    • Doppelbilder.

  • Wirksamkeit und Wirkdauer bleiben selbst nach mehrjähriger Therapie unverändert erhalten.
