In jüngster Vergangenheit wurde die Kontroverse über die Notwendigkeit und die Art der Darmvorbereitung vor kolorektalen Resektionen wieder neu belebt. Neben der obligaten intravenösen Antibiotikaprophylaxe (IVAB) könnten die mechanische Darmreinigung (mechanical bowel preparation – MBP) und die präoperative orale Antibiotikagabe (OAB) potenziell effektive Instrumente der Darmvorbereitung sein. Ziel aller Maßnahmen ist die Reduktion von Infektionen im Operationsgebiet („surgical site infections“ – SSI) oder Anastomoseninsuffizienzen. Im Folgenden werden strukturiert die Ergebnisse der vorliegenden systematischen Literaturrecherchen mit nachfolgender Metaanalyse randomisierter kontrollierter Studien zur Bedeutung der präoperativen Darmvorbereitung vor elektiven kolorektalen Resektionen dargestellt.
