Die unerwartet positive Kreuzprobe

Zusammenfassung

Eine positive Verträglichkeitsprobe bei scheinbar negativem Antikörperstatus ist ein seltenes Ereignis, das jedoch häufig eiliger Aufklärung bedarf. Zu den Ursachen gehören Verwechslungen von Proben oder Patienten, Fehlbestimmungen, neu nachweisbare irreguläre Alloantikörper, Antikörper gegen niedrigfrequente Antigene, gegen Reagenzienbestandteile oder gegen das Merkmal A₁. Ein standardisierter Arbeitsfluss hilft, die Ursachen rationell und schnell aufzuklären und so eine sichere Versorgung des Patienten zu ermöglichen.

Abstract

An incompatible serologic RBC crossmatch without prior antibody detection occurs infrequently and mostly as a surprise in immunohematologic routine laboratories. Causes are misidentification of samples or patients, invalid test results, newly detectable alloantibodies, antibodies directed against low frequency antigens, against reagent components or against the A subgroup A₁. Following a standardized workflow is helpful in solving the problem quickly and efficiently and thus ensuring safe and immediate patient care.
