“If you do not change, you may become extinct” [[9]].

Over the past 50 years paediatric surgery has developed into a generally accepted, independent speciality in nearly all European countries. The future organisation and work of paediatric surgery needs to be adapted to the rapid changes in society (i.e. the declining birth-rate), the economic restrictions on healthcare, the regulations of European Union (bodies like the UEMS) and the unforeseeable, rapid development of medical technology.

This letter draws attention to those factors which are expected to determine and/or modify the further development of paediatric surgery in Europe in the 21st century.
