Die Sichtweise auf die Pathogenese der Parodontitis hat sich in den letzten Jahren deutlich verändert. Während man früher von einem rein infektiologischen (mikrobiologischen) Problem ausging (Plaquehypothese), wird heute die Parodontitis als eine chronisch entzündliche Erkrankung angesehen, die in ihrer Entstehung und Progression multifaktoriell bedingt ist. Auch wenn die Plaquebildung neben exogenen Risikofaktoren wie Rauchen, Stress und mangelnder Mundhygiene weiterhin den wichtigsten Kausalfaktor darstellt, weiß man heute, dass die individuelle Immunantwort eines Patienten entscheidend dafür ist, welche Folgen dieser auslösende Reiz (Biofilm) für den Entzündungsprozess hat.
