Atypical metastatic pattern of prostate cancer detected with 68Ga-PSMA PET/CT

Introduction

PSMA-ligand PET/CT is a promising imaging modality for prostate cancer (PC), and has rapidly established itself as the modality of choice in the re-staging of biochemical recurrence. Nevertheless, in recent years an increasing number of case reports have been published describing incidental and non-prostatic lesions of both benign and malignant origin, which exhibit high PSMA radiotracer uptake [1]. This frequently leads to confusion in the assessment of PSMA-positive lesions that are not located within typical metastatic sites of PC.

The present case demonstrates that PC metastases should be considered even in non-classical patterns, owing to the high positive predictive value of PSMA-ligand-imaging [2]. The patient benefited from accurate staging of his disease, resulting in an early change in his management.
