Kernaussagen
Für ein Verständnis des histologischen Erscheinungsbildes und der Biologie der Mammakarzinome ist die Kenntnis der bimodalen Differenzierung des Mammaepithels Voraussetzung. So weist die Mamma 2 Epitheltypen auf, die sich wahrscheinlich von einer gemeinsamen Progenitorzelle ableiten: eine die Lobuli und Milchgänge auskleidende luminale Epithelschicht und eine Myoepithelschicht, die die luminale Zellschicht umscheidet und kontraktile Elemente enthält. Die Mehrzahl der häufigen Mammamalignome (duktale und lobuläre Karzinome) geht von luminalen Zellen aus bzw. weist eine entsprechende Differenzierung auf. Viel seltener sind hingegen Neoplasien basierend auf einer myoepithelialen Differenzierung bzw. Herkunft. Sie sind pathohistologisch auch wesentlich schwieriger zu diagnostizieren. Ausgehend von den oben beschriebenen Differenzierungsrichtungen werden die verschiedenen Mammatumoren unter Berücksichtigung der Histologie klassifiziert. Die Beschreibung orientiert sich dabei an der Klinik, der bildgebenden und bioptischen Diagnostik, der Pathomorphologie und dem biologischen Verhalten sowie den Prognosefaktoren.
