Zusammenfassung

Studienziel: Nach wie vor ist der Stellenwert der Arthroskopie bei Gonarthrose umstritten. Neue Untersuchungen zeigten, dass eine Arthroskopie keinen größeren Nutzen bei Gonarthrose hat als eine Sham-Operation. Andere Studien kommen jedoch zu gegenteiligen Ergebnissen. Der richtigen Auswahl der Patienten, scheint eine Schlüsselrolle zum Erfolg der Arthroskopie bei Gonarthrose zu zukommen. Das Ziel dieser retrospektiven Untersuchung war die Identifizierung von Faktoren, die ein positives Outcome nach Arthroskopie bei Gonarthrose erwarten lassen. Methode: In einer retrospektiven Analyse wurden 49 Patienten im Alter von über 65 Jahren im Mittel 2,5 Jahre nach Kniearthroskopie untersucht. Zur Bewertung wurde der modif. Lysholm Score verwendet. Er beinhaltet subjektive Angaben des Patienten, eine klinische Nachuntersuchung und eine radiologische Beurteilung. Ergebnisse: Die Ergebnisse des Lysholm Score zeigten keinen Zusammenhang mit dem Patientenalter und -geschlecht. Es fand sich ein statistisch signifikant schlechteres Ergebnis bei den Patienten mit einer Beinachsenfehlstellung und bei den Patienten, die im Zeitraum zwischen Kniearthroskopie und Nachuntersuchung ein radiologisches Voranschreiten der Gonarthrose erfahren hatten, auch wenn sie insgesamt einen radiologisch geringeren Arthrosegrad aufwiesen. Schlussfolgerung: Die richtige Patientenauswahl ist ein entscheidendes Kriterium für den postoperativen mittelfristigen Erfolg der Arthroskopie bei Gonarthrose. Neben der Beinachse spielt der Aktivitätsgrad der Arthrose zum Zeitpunkt der Arthroskopie eine wichtige Rolle für den Erfolg der Operation.
