In der Behandlung der bipolaren Erkrankung ist Lithium mit antimanischer, antidepressiver und antisuizidaler Wirkkomponente der „Goldstandard”.

Die Therapie akuter Episoden ist zugleich der erste Schritt in die Rezidivprophylaxe, was bei der Auswahl des Medikamentes bedacht werden sollte. Die Indikation zur Rezidivprophylaxe sollte frühzeitig gestellt werden.

Besonders klassische Bipolar-I-Erkrankungen sprechen gut auf Lithium an. Bei atypischer Symptomatik und Rapid Cycling stellen die Antikonvulsiva Valproat und Carbamazepin wertvolle Alternativen dar. Valproat zeigt eine gute antimanische Wirksamkeit und ist, wie Olanzapin, Mittel der ersten Wahl bei gemischten Episoden. Lamotrigin hat seinen Wirkschwerpunkt in der Therapie und insbesondere Prävention depressiver Episoden.

Atypische Neuroleptika gewinnen in der Monotherapie des manischen Syndroms und in der Kombinationstherapie zur Verbesserung von antidepressiver und phasenprophylaktischer Wirkung an Bedeutung.

Der Einsatz von Antidepressiva zur Therapie depressiver Episoden sollte aufgrund der Gefahr von Manie-Induktion und Phasenbeschleunigung nur in Verbindung mit Stimmungsstabilisierern erfolgen und zeitlich begrenzt sein, wobei SSRI Mittel der ersten Wahl sind. Besonders bei Bipolar-I-Patienten ist auf einen Manieschutz zu achten. Bei Rapid Cycling sollte generell auf Antidepressiva verzichtet werden.

In der Langzeitbehandlung ist eine Darstellung mittels Life-Chart-Methodik, idealerweise in elektronischer Form, unentbehrlich.

Bei Verdacht auf Prophylaxeresistenz muss eine Pseudoresistenz ausgeschlossen und die Serumspiegel bestimmt und optimiert werden. In der Praxis ist eine wirkungsvolle Phasenprophylaxe häufig eine Kombinationstherapie mit Stimmungsstabilisierern, wobei auf Interaktionen geachtet werden muss. Die beste Datenlage besteht für Carbamazepin plus Lithium. Aufgrund der unterschiedlichen Wirkschwerpunkte erscheint die Kombination von Lithium und Lamotrigin vielversprechend. Führt eine Zweierkombination nicht zum Erfolg, kann eine Dreierkombination versucht werden. Eine weitere Möglichkeit zur Durchbrechung der Prophylaxeresistenz stellt die Gabe von L-Thyroxin in supraphysiologischer Dosierung dar.
