Zusammenfassung

Einleitung: Primäre Duodenalkarzinome sind selten und die Diagnose wird oft erst in einem fortgeschrittenen Stadium gestellt. Der Stellenwert der chirurgischen Therapie stützt sich auf Erfahrungsberichte einzelner Kliniken mit geringen Fallzahlen. Wir berichten unsere Ergebnisse in der chirurgischen Therapie des primären Duodenalkarzinoms. Methoden: Zwischen 1992 und 2003 wurden 11 Patienten wegen eines primären Duodenalkarzinoms operiert. Patienten mit Papillenkarzinomen wurden ausgeschlossen. Symptome, Diagnostik, Therapie und Nachbeobachtung wurden retrospektiv ausgewertet. Ergebnisse: Das mediane Alter bei Diagnosestellung betrug 68 (Spannbreite 51-80) Jahre. Es waren 6 Frauen und 5 Männer betroffen. Die häufigsten Symptome waren Oberbauchschmerzen 55 % (6/11), Anämie 46 % (5/11) und Gewichtsverlust 27 % (3/11). Das Intervall zwischen den ersten Symptomen und der Therapie betrug im Mittel 5 (Spannbreite 0-17) Monate. Bei 5 dieser 11 Patienten wurde die Diagnose primär endoskopisch gestellt. Bei 2 Patienten mit initial unauffälligem endoskopischen Befund wurde in der Doppelkontraströntgenuntersuchung (MDP) ein tumorsuspekter Befund erhoben. Bei 36 % (4/11) der Patienten war der Tumor in der Pars superior, bei 18 % (2/11) in der Pars descendens und bei 46 % (5/11) in der Pars horicontalis bzw. Pars ascendens duodeni lokalisiert. Es wurden 8 Operationen nach Kausch-Whipple und 3 Duodenumsegmentresektionen durchgeführt. Es konnte in allen Fällen eine R0-Resektion erzielt werden. Gemäß der UICC-Klassifikation hatte ein Patient ein Stadium-I-, vier Patienten ein Stadium-II-, vier Patienten ein Stadium-III- und ein Patient ein Stadium-IV-Duodenalkarzinom. Bei einem Patienten konnte aufgrund eines unklaren Lymphknotenstatus keine Klassifikation erfolgen. Nach einer medianen Nachbeobachtung von 32 Monaten (Spannbreite 9-110 Monate) leben 4 Patienten (110, 42, 39 und 9 Monate) ohne Hinweis auf Rezidiv oder Metastasen. Schlussfolgerung: Die Diagnose eines primären Duodenalkarzinoms wird aufgrund unspezifischer Symptome oft erst in einem fortgeschrittenen Stadium gestellt. Bei klinischem Verdacht und unauffälligem Endoskopiebefund kann eine Doppelkontraströntgenuntersuchung des Duodenums hilfreich sein. Durch die aggressive chirurgische Therapie lassen sich in Einzelfällen lange rezidiv- und metastasenfreie Intervalle erzielen.

Abstract

Background: Primary adenocarcinoma of the duodenum is a rare disease often diagnosed at an advanced stage. The significance of surgical resection relies on single center experiences with often only few cases. We report our results in surgical treatment of primary duodenal adenocarcinoma. Methods: Between 1992 and 2003 11 patients underwent surgical treatment for primary adenocarcinoma of the duodenum. Patients with periampullary carcinoma were excluded. We retrospectively reviewed presenting symtoms, diagnostic procedures, perioperative outcome and follow-up. Results: The median age was 68 (51-80) years. There were 6 women and 5 men. The most frequent symptoms were abdominal pain 55 % (6/11), anaemia 46 % (5/11) and weight-loss 27 % (3/11). The duration of symptoms prior to diagnosis was 5 months in average ranging from 0-17 months. In 5 of the 11 patients diagnosis was primarily made on endoscopy. In two patients primary endoscopy failed to detect the tumor. Both patients did show a suspicous result on barium swallow series though. In 36 % (4/11) of patients the tumor was located in pars superior, in 18 % (2/11) in pars descendens and in 46 % (5/11) in pars horicontalis and ascendens of the duodenum. We performed 8 Whipple procedures and 3 segmental duodenal resections. In all cases R0-resection could be archieved. Based on the TNM-staging 1 patient had stage I, 4 patients stage II, 4 patients stage III and 1 patient stage IV disease. One patient could not be classified due to an unclear nodal status. After median follow-up of 32 months (9-110 months) 4 patients are still alive (110, 42, 39 and 9 months) without any signs of relapse or metastases. Conclusion: Diagnosis of primary duodenal adenocarcinoma is often made at an advanced stage due to non-specific symptoms. In case of clinical suspicion and inconspicuous endoscpoy a barium swollow series can be helpfull. With an aggressive surgical approach long-term survival can be archieved in some cases.
