Zusammenfassung
Das Heyde-Syndrom bezeichnet die Koinzidenz von Aortenklappenstenose und intestinalen Blutungen bei Angiodysplasien. Ein Sistieren der intestinalen Blutungen nach Durchführung eines Aortenklappenersatzes wurde beschrieben. Kasuistik: Wir beschreiben den Krankheitsverlauf einer 75-jährigen Patientin mit rezidivierenden intestinalen Blutungen, die schließlich auf Angiodysplasien zurückgeführt werden konnten. Nach Ersatz einer hochgradig stenosierten Aortenklappe durch eine Bioprothese kam es zum nachhaltigen Sistieren der Blutungen. Schlussfolgerung: Rezidivierend intestinale Blutungen bei Angiodysplasien sollten an die Möglichkeit einer gleichzeitig bestehenden Aortenklappenstenose denken lassen.
Abstract
Angiodysplasia are common in patients over the age of 60. Heyde syndrome describes the coincidence of aortic valve stenosis and gastrointestinal bleeding from angiodysplasia. We describe one characteristic case of aortic valve stenosis and gastrointestinal bleeding from angiodysplasia which subsided after replacement with an aortic valve bioprothesis. We review the current literature and discuss the actual explanation approaches for this phenomenon. Conclusion: There seems to be a clear indication for valve replacement in the case of aortic valvestenosis and gastrointestinal bleeding due to angiodysplasia.
