Ultrasonographic Findings in Patients with Olecranon Bursitis

Zusammenfassung

Hintergrund: Die Sonographie des muskuloskeletalen Systems stellt ein wichtiges Bild gebendes Verfahren bei der Diagnose der Bursitis olecrani insbesondere im frühen Stadium dar. Sie erlaubt den sicheren Nachweis kleiner Flüssigkeitsmengen sowie die Differenzierung zwischen knöchernen und Weichteilveränderungen. Mit der Sonographie lassen sich Ergüsse, eine Proliferation der Synovia, Kalzifikationen, freie Gelenkkörper, Rheumaknoten, Gichttophi und septische Prozesse darstellen. Ziel: Untersuchung der Rolle der Sonographie bei Diagnose und Behandlung von Patienten mit Bursitis olecrani. Methoden: 34 Patienten mit einer Schwellung über dem Olecranon wurden sonographisch untersucht. Die asymptomatische Gegenseite diente als Kontrollgruppe. Ergebnisse: Bei 20 Patienten fanden wir eine Zunahme der Flüssigkeit in der Bursa olecrani. In 5 Fällen wurde eine Proliferation der Synovia nachgewiesen, bei 2 freie Gelenkkörper, bei 5 weiteren ein deutlich erhöhter Blutfluss als Hinweis auf eine Entzündung. 2 Patienten hatten eine Tendinitis der Trizepssehne mit Kalzifikationen. Schlussfolgerung: Die Sonographie stellt ein extrem effektives Instrument bei der Diagnostik von Weichteilveränderungen im Bereich des Olekranums dar.

Abstract

Background: Musculoskeletal ultrasonography (U.S.) is an important imaging technique in the diagnosis of olecranon bursitis, especially for early manifestation. It allows sensitive detection of small fluid collections as well as for differentiation between soft tissue and bone lesions. U.S. examination allows detection of effusions, synovial proliferation, calcifications, loose bodies, rheumatoid nodules, gout tophi and septic processes. Aim: To assess the role of ultrasonography in the diagnosis and management of patients with olecranon bursitis. Methods: Ultrasound was used in 34 patients with swelling above the olecranon. The opposite asymptomatic side served as a control group. Results: 20 patients demonstrated increased fluid collection in the olecranon bursa. 5 had synovial proliferation, 2 cases showed loose body, 5 patients revealed markedly increased blood flow consistent with inflammation, 2 patients had triceps tendonitis with calcifications. Conclusions: Sonography is an extremely effective tool for the diagnosis of soft tissue lesions in the olecranon area.
