Aktuelle Diagnostik und Therapie der Hepatitis C

Bei Verdacht auf das Vorliegen einer chronische Hepatitis C ist der Anti-HCV-Test das klassische Suchverfahren.

Bei positivem Anti-HCV erfolgt die direkte Bestimmung der HCV-RNA zur Unterscheidung zwischen einer chronischen oder ausgeheilten Infektion.

HCV-Quantifizierung und HCV-Genotypisierung sollten vor Therapieeinleitung erfolgen, da sie mit der Ansprechrate korrelieren und der Festlegung der Therapiedauer dienen.

Die akute Hepatitis C wird derzeit mit Interferon-α als Monotherapie für 24 Wochen therapiert.

Die Standardtherapie der chronischen Hepatitis C besteht aus pegyliertem Interferon in Kombination mit Ribavirin.

Patienten mit einer chronischen Hepatitis-C-Genotyp-1-Infektion werden über 48 Wochen behandelt. Ribavirin sollte dabei körpergewichtsbezogen dosiert werden.

Patienten mit einer chronischen Hepatitis-C-Genotyp-2- oder -3-Infektion werden über 24 Wochen behandelt. Ein Vorteil einer Ribavirindosis von über 800 mg/d ist nicht belegt.

Patienten mit Rückfall oder Nonresponse auf eine Interferon-α-Monotherapie sollten kombiniert mit einem pegylierten Interferon und Ribavirin behandelt werden.

Eine erneute Therapie von Patienten mit Rückfall auf eine Kombinationstherapie mit einer erneuten Kombinationstherapie ist gerechtfertigt. Für Patienten mit Nonresponse nach einer Kombinationstherapie gibt es zurzeit keine allgemein gültigen Empfehlungen. Die Behandlung sollte im Rahmen kontrollierter Studien erfolgen.

Ein vorzeitiger Therapieabbruch ist indiziert bei Patienten mit chronischer Hepatitis-C-Genotyp-1-Infektion, wenn die Viruslast in Woche 12 nicht mindestens um 2 log-Stufen abgefallen ist, bzw. wenn die absolute HCV-RNA-Konzentration zu Woche 12 über 30000IU/ml liegt, sowie wenn zu Woche 24 der Therapie HCV-RNA (>50IU/ml) im Blut noch qualitativ nachweisbar ist.
