Chirurgische Funktionsverbesserung an der oberen Extremität von Tetraplegikern

Zusammenfassung

Bei Patienten mit Verletzung des Rückenmarks im Halsbereich besteht neben der vollständigen Lähmung der unteren Extremität zumindest ein teilweiser Funktionsausfall der oberen Gliedmaßen (traumatische Tetraplegie), der mit dramatischen Einschränkungen verbunden ist. Seit über 30 Jahren wird an einigen Zentren versucht, vor allem durch Sehnen-Transpositionen, die Arm- und Handfunktion dieser Patienten chirurgisch zu verbessern. Wir berichten über unsere Erfahrungen bei 69 tetraplegischen Patienten, bei denen wir in den letzten zwölf Jahren 188 Eingriffe durchgeführt haben. In allen Fällen konnte eine funktionelle Verbesserung erzielt werden. Bei keinem einzigen Patienten ist es zu einer Verschlechterung gekommen. Die Mehrheit der Patienten war mit dem erreichten Ergebnis zufrieden.

Abstract

Cervical spinal cord injuries represent about 40 % of all spinal cord lesions. In addition to the complete paralysis of the lower limbs they induce a dramatic decrease of upper limb function. Surgical rehabilitation of the upper limbs, based mainly on tendon transposition procedures, was advocated by only few surgeons more than thirty years ago and has developed in recent years in view of their satisfactory results. We report a consecutive series of 69 tetraplegic patients operated during the past twelve years, amounting to 188 rehabilitation procedures on their upper limbs. Our results indicate that surgery was beneficial in all cases. There was no case of functional loss and the patients were mostly satisfied with the procedures.
