Misteltherapie - evidenzbasierte komplementäre Maßnahme zur Chemotherapie

Zusammenfassung

Standardisierte Mistelextrakte stehen unter den komplementären Behandlungsmethoden von Krebspatienten seit mehreren Jahrzehnten an erster Stelle. Seit dieser Zeit werden viele positive Erfahrungen von Krebspatienten und von behandelnden Ärzten berichtet. Doch nun stehen öffentlich zugängliche klinische Daten einer in Anlehnung an ICH-GCP (Good-Clinical-Practice-Richtlinien der International Conference on Harmonisation of Technical Requirements for Registration of Pharmaceuticals for Human Use (ICH)) durchgeführten prospektiv kontrollierten randomisierten Multicenterstudie zur Verfügung. 233 Patienten mit Brustkrebs (n = 68), Ovarialkrebs (n = 71) und kleinzelligem Lungenkrebs (NSCLC, n = 94) wurden in diese Studie eingeschlossen. 224 Patienten gelangten zur finalen Analyse (n = 115 mit standardisiertem Mistelextrakt HELIXOR^®, n = 109 Kontrollgruppe mit dem Immunmodulans Lentinan). Alle Patienten erhielten eine Standardchemotherapie und wurden komplementär entweder mit standardisiertem Mistelextrakt oder Lentinan gemäß Studienprotokoll behandelt. Die Lebensqualität konnte signifikant für die Patienten aus der Mistelgruppe verbessert werden (p < 0,05). Dieses Resultat wurde über die Auswertung von drei unterschiedlichen Indizes (FLIC = Functional Living Index: Cancer, TCM-Score = Traditional Chinese Medicine Score und KPI = Karnofsky Performance Index) erreicht. Auch die Reduktion der unerwünschten Ereignisse von 92 auf 50 Ereignisse sowie die Reduktion schwerwiegender unerwünschter Ereignisse von 10 auf 5 war signifikant.

Diese Studie zeigt, dass komplementäre Behandlung von Tumorpatienten mit standardisierten Mistelextrakten die Nebeneffekte einer Chemotherapie deutlich reduzieren kann und die Lebensqualität signifikant erhöht.

Abstract

Standardized aqueous mistletoe extracts have been applied to cancer patients for several decades as complementary medicine. However, the evidence of these results is controversial because of the problem of adequate methodology in evaluating efficacy. In this context a multicentric, randomized, open, prospective clinical trial was conducted. Following the guidelines of Good Clinical Practice (GCP) this study was performed to get information on efficacy, safety and side-effects of the standardized mistletoe extract (sME). Two hundred and thirty-three patients with breast (n = 68), ovarian (n = 71) and non-small cell lung cancer (NSCLC; n = 94) were enrolled into this study. Two hundred and twenty-four patients fulfilled the requirements for final analysis (n = 115 treated with sME HELIXOR^® A; n = 109 comprising the control group being treated with the approved immunomodulating phytopharmacon Lentinan). All patients were provided with standard tumor-destructive treatment schedules and complementarily treated with sME or Lentinan during chemotherapy according to treatment protocol. Quality of life (QoL) was significantly (p < 0.05) improved for patients that were complementarily treated with sME, as determined by the indices FLIC (Functional Living Index-Cancer), TCM (Traditional Chinese Medicine Index) and the KPI (Karnofsky Performance Index) in comparison to the control group. Additionally, the occurrence of adverse events (AEs) was less frequent in the sME than in the control group (total number of AEs 52 versus 90 and number of serious AEs 5 versus 10 in study and control group, most of them due to chemotherapy).

This study showed that complementary treatment with sME can beneficially reduce the side-effects of chemotherapy in cancer patients and thus improve quality of life.
