[[author photo]]The perception of the changes caused by physical and biologic denominators, such as gravity, ultraviolet, and cellular aging over the face, has evolved from a two-dimensional to a three-dimensional interpretation over the last 2 decades in aesthetic surgery. Realization of the volume descensus along with resorption and matrix deformation also helped us to reevaluate the interrelations of subcompartments of the face during the aging process. Periorbital and perioral areas are the best two examples where we see this complex interaction within the same effector system. Thus, addressing the problems and solutions for these areas developed the utilization of the global and compartmentalized approaches sequentially or simultaneously. Perioral problems can be addressed through a more global approach such as volumetric reorganization and elevation of the whole midface unit or through a locally targeted approach such as augmentation, suspension, or reduction of the perioral elements or through a combination of these two approaches. In this issue of Seminars in Plastic Surgery we aim to discuss these approaches in a differential pattern, as in isolated maneuvers or as combinational approaches.

The right answer is always in an analytical approach, which utilizes different combinations of these maneuvers in a pattern-specific or even a case-specific modality.

The articles are presented according to the sequence of the addressed tissue compartment from bone to the surface.
