Zusammenfassung.
Die Entwicklung postoperativer, peritonealer Adhäsionen lässt sich durch laparoskopische
Methoden, gewebeschonende Techniken und puderfreie Handschuhe allein nicht signifikant
reduzieren. Epidemiologische Daten zeigen mit Zunehmen der medizinischen Versorgung
sowie der Lebenserwartung einen Anstieg adhäsions-bedingter Komplikationen und der
damit einhergehenden sozio-ökonomischen Belastung. Derzeit steht für die klinische
Routine in der Viszeralchirurgie kein Konzept oder Medikament zur zuverlässigen Adhäsionsprophylaxe
zur Verfügung. Ergebnisse der experimentellen Forschung geben jedoch Anlass zu der
Hoffnung, dass sich mittelfristig eine relevante Reduktion postoperativer Verwachsungen
erreichen lassen wird.
Incidence, complications and prophylaxis of postoperative, peritoneal adhesions.
Laparoscopic approach, meticulous technique, and powder-free gloves alone are not
capable to reduce formation of postoperative peritoneal adhesions to a significant
degree. Epidemiologic data show rising figures of adhesion related complications and
subsequent socio-economic expenditures based on an increase of available medical services
and life expectancy. As of today, there is neither a regimen nor a drug available
for reliable prevention of adhesions in the clinical routine of visceral surgery.
Results of experimental research, however, raise hopes that a relevant reduction of
adhesions will be achieved in the medium-term future.
