Die arthroskopische Therapie frischer Discus articularis-Verletzungen bei distalen Radiusfrakturen[1]

Zusammenfassung

Verletzungen des ulnokarpalen Komplexes (TFCC) werden immer häufiger sekundär nach Radiusfrakturen festgestellt. Eine primäre Diagnostik und Therapie dieser Verletzungen wären wünschenswert. Die folgende Arbeit stellt erstmals gesondert Techniken und Ergebnisse zur primären Therapie von TFCC-Verletzungen nach Radiusfrakturen vor. Von Januar 1998 bis September 1999 wurden insgesamt 22 Patienten mit frischen TFCC-Verletzungen arthroskopisch versorgt. Ein Patient hatte eine Skaphoidfraktur, kombiniert mit einer Palmer-1B-Verletzung des TFCC. 21 Patienten hatten intraartikuläre Radiusfrakturen vom Typ C nach der AO-Klassifikation. Die Palmer-1A- (n = 5) und 1C- (n = 4) Verletzungen wurden wie die Verletzung des Meniscus ulnocarpalis (n = 1) arthroskopisch debridiert. Die Palmer-1B-Verletzungen wurden in einer „Outside-Inside“-Technik (n = 6) und in einer neuen „All-Inside“-Technik (n = 6) versorgt. Palmer-1D-Verletzungen (n = 5) wurden nach der Fellinger-Technik versorgt. Alle Patienten konnten sechs bis 15 Monate nach der Operation klinisch nachuntersucht werden. Die Umwendbewegung war nur gering eingeschränkt. Typische Symptome der Diskuspathologie fanden sich nur bei einem Patienten nach „Outside-Inside“-Refixation einer Palmer-1B-Verletzung. Im MRT zeigte sich in diesem Fall eine Diskusinstabilität. Insbesondere Patienten, die mit der neuen „All-Inside“-Technik versorgt worden waren, waren beschwerdefrei. Die arthroskopische Versorgung von frischen TFCC-Verletzungen ist technisch möglich und mit guten Ergebnissen durchführbar. Besonders „All-Inside“-Techniken zur Diskusrefixation sind zeit- und damit auch kostensparend und minimal invasiv.

Summary

The primary simultaneous diagnosis and therapy of TFCC tears in distal radius fractures is still the exception. We present our results of arthroscopic treatment of these injuries. From January 1998 until September 1999, we treated 21 patients with a type C fracture (AO classification) of the distal radius and one patient with a scaphoid fracture and TFCC tears. The Palmer 1A (n = 5) and 1 C (n = 4) tears have been arthroscopically shaved as well as the meniscal tear (n = 1). The Palmer 1B tears were refixed either in an outside-inside technique (n = 6) or in a new all-inside technique (n = 6). Palmer 1D tears were refixed in the Fellinger technique (n = 5). In the clinical follow-up examination six to fifteen months later, we saw symptoms of ulnar-sided wrist pain only in one case of a Palmer 1B tear treated in an outside-inside technique. Especially the patients treated by the new all-inside technique were free from symptoms of TFCC instability. We conclude, that arthroscopic treatment of TFCC tears in acute radius fractures is possible with good results. The new technique of all-inside repair is minimally invasive and shows good results.
