Treatment rationale for first-time anterior shoulder dislocation

Summary

The rapid development of arthroscopic techniques has raised questions on the treatment of first shoulder dislocations. Primary arthroscopic or even open labral repair have been recommended. However, the available prospective studies demonstrate a recurrence rate of not more than 50 % in the global population. This number increases steeply if certain risk factors are present: young age (under 25 years), shoulder sports, adequate trauma with acute unidirectional instability. For this selected group, arthroscopic labral repair should be proposed and should be performed by a surgeon specifically trained in this advanced arthroscopic technique. Repair of the labrum is feasable with various arthroscopic techniques, whereas the shortening of redundant capsular areas still poses problems. The efficiency of laser- or heat-shrinking of the shoulder capsule must yet be proven.

Zusammenfassung

Neue diagnostische und therapeutische Angebote haben zu einer intensiven Diskussion über das Vorgehen bei der Erstluxation der Schulter geführt. Die Empfehlungen differieren dabei von rein konservativer Therapie über arthroskopische Maßnahmen bis zu offenen Operationsverfahren. Wesentliche Entscheidungskriterien sind einerseits die Rezidivrate und andererseits zu erwartende funktionelle Einschränkungen des Schultergelenks. Das Reluxationsrisiko liegt in unselektionierten Patientenkollektiven bei ca. 50 %, steigt aber bei Vorliegen bestimmter Risikofaktoren (Alter unter 25 Jahre, Schultersport, adäquates Ersttrauma, unidirektionale Instabilität, keine Hyperlaxität, große Hill-Sachs-Delle, erhebliche Instabilität) steil an. Besteht ein hohes Reluxationsrisiko und liegt ein Therapiewunsch vor, so ist heute auch bei der Erstluxation die arthroskopische Labrumrefixation indiziert. Berücksichtigt werden sollte jedoch, daß diese Methode bisher nur in den Händen sehr erfahrener arthroskopischer Operateure gute Ergebnisse liefert und derzeit die Möglichkeiten der Verkleinerung des Kapselvolumens auf arthroskopischem Wege noch begrenzt sind. Somit ergibt sich heute für die Erstluxation der Schulter ein differenziertes verletzungs- und risikoadaptiertes Therapiekonzept.
