A Psychosomatic Dilemma

Summary

Relationships with patients are one of the most rewarding aspects of the clinical course, and have taught me more about the art of medicine than any textbook ever could. I always regard it as a great privilege when a complete stranger puts their trust in me, and shares with me their stories, their feelings, and even sometimes a closely-guarded secret. I have sometimes found it distressing too, when people tell me what they have been through. It can be difficult to know how to express my empathy and pity in an appropriate way - especially with psychiatric patients. Of course, mot illnesses are influenced by both physical and psychological factors, but it is more comfortable if one can dissect these apart with the scalpel of the intellect. The following case I found very challenging, because mind and matter seemed inseparably intertwined. I was uncomfortable with both the doctors’ and the patient’s attitudes towards body-mind interactions.

Zusammenfassung

Die Beziehung zum Patienten ist einer der lohnendsten Aspekte im klinischen Alltag und hat mich mehr über die ärztliche Kunst gelehrt als es ein Lehrbuch je könnte. Ich empfinde es immer als ein großes Privileg, wenn ein völlig Unbekannter mir Vertrauen schenkt, mir seine Geschichte anvertraut, seine Gefühle und manchmal sogar ein gutbewachtes Geheimnis. Manchmal bekümmert es mich auch, zu hören, was der eine oder andere durchgemacht hat. Es kann sehr schwer sein, sein Mitgefühl und Mitleid in angebrachter Weise zum Ausdruck zu bringen - besonders bei psychiatrischen Patienten. Natürlich werden die meisten Erkrankungen sowohl von physischen als auch psychischen Faktoren beeinflusst, aber es ist bequemer, wenn es einem gelingt, diese Faktoren mit dem Skalpell des Intellekts auseinander zu dividieren. Der folgende Fall war für mich eine große Herausforderung, da Geist und Körper untrennbar miteinander verbunden schienen. Ich fühlte mich mit der Einstellung sowohl der Therapeuten als auch des Patienten den Körper-Geist-Interaktionen gegenüber unwohl.
