Einfluss von chronisch-entzündlichen Darmerkrankungen auf den Ernährungszustand

Konsens

Als Folge der Colitis ulcerosa sind sowohl globale Malnutrition als auch spezifische Mangelzustände beschrieben (B). Es liegen jedoch keine Daten vor, die einen Einfluss der Krankheitsaktivität auf den Ernährungszustand belegen (B). An spezifischen Mangelzuständen sind Anämie, Eisenmangel und Folsäuremangel beschrieben (B). Ein Selenmangel, Kalziummangel oder allgemeiner Mangel an Antioxydanzien ist nicht beschrieben. Spezifische Mangelzustände werden wesentlich auch durch die Therapie, zum Beispiel durch Sulfasalazin, bedingt (B).

Erläuterung

Die Angaben über die globale Mangelernährung bei Colitis ulcerosa beziehen sich Im Wesentlichen auf Fallberichte. Epidemiologische Studien, die die Prävalenz von Untergewicht bzw. Gewichtsverlust genauer abschätzen lassen, fehlen. Gewichtsverluste werden im akuten Schub beobachtet. Hinweise auf Gewichtsverluste in der Remission liegen nicht vor. Spezifische Angaben über Veränderungen der Körperzusammensetzung durch Colitis ulcerosa, d. h. Relation von Proteinverlust bzw. Fettverlust, liegen nicht vor [1].

Ähnlich ist die Situation bezüglich der spezifischen Mangelzustände. Eine Anämie, definiert als Hämoglobin unter 10 g/dl, wurde bei 37 % der Patienten in einer definierten Gruppe gefunden [2], Eisenmangel bei 55 % und Zinkmangel bei 10 % [3]. Epidemiologische Studien über die Prävalenz dieser Mangelzustände bei C. U. liegen ebenfalls nicht vor. Gut gesichert ist der Einfluss von Sulfasalazin auf die Folsäureresorption. Unter Sulfasalazintherapie wurde Folsäuremangel mehrmals beschrieben [4] [5]. Eine erniedrigte Knochendichte liegt bei Colitis ulcerosa nicht vor [6] [7]. Ein Selenmangel oder allgemeiner Antioxidanzienmangel liegt ebenfalls nicht vor.

Bezüglich der Ursache für spezifische Mangelzustände kam die Konsensuskonferenz dahingehend überein, dass Mangel an Spurenelementen und Vitaminen bei Colitis ulcerosa krankheitsbedingt keine relevante Rolle spielen, sondern wesentlich durch die Therapie bestimmt werden, zum Beispiel Folsäuremangel durch Sulfasalazineinnahme. Zur Bestimmung des Folsäurestatus ist bei Verdacht auf Folsäuremangel die Bestimmung in Erythrozyten notwendig, normale Plasmawerte schließen einen Mangel nicht aus.
