Ursache eines akuten Abdomens

Zusammenfassung

Geschildert wird der Fall eines 69-jährigen Patienten, der wegen Übelkeit, Erbrechen und heftigsten epigastrischen Schmerzen einen Notarzt anfordert. Der Transport in die Klinik verläuft unproblematisch, dort entwickelt sich innerhalb der nächsten Stunden ein linksseitiges Pleuraempyem. Auffallend ist zudem ein Anschwellen der Halsweichteile und ein massiver Anstieg des C-reaktiven Proteins von 0,4 mg/dl auf maximal 39,6 mg/dl. Nachdem die häufiger vorkommenden Ursachen für ein akutes Schmerzsyndrom der Oberbauch- und Thoraxorgane ausgeschlossen werden konnten, lautete die Diagnose letztendlich auf spontane Ösophagusruptur mit Pleura- und Mediastinalempyem.

Boerhaave's Syndrome - A Rare Reason Causing Acute Abdominal Pain

Reported is the case of a 69 year old male who called the emergency medical service due to nausea, vomiting and severe epigastric pain. The transport to the hospital was without any problems. Within a few hours a left hand side pleuraempyema developed. Lateron there was throat swelling and a rapide increase of c-reactive protein from 0.4 mg/dl to 39.6 mg/dl. After exluding the common diseases causing acute abdominal and thoracic pain the final diagnosis was spontaneous rupture of esophagus associated with empyema of pleura and mediastinum.
