A 18-year-old boy with a recent history of NSAID use (for 3 days one week ago) was admitted to the hospital with suspected upper gastrointestinal bleeding, presenting with melena, large-volume enterorrhagia, deterioration of general condition, pallor, and sweating, along with hemodynamic instability, and was transferred to the ICU. After receiving 3 units of packed red blood cells and clinical management to improve shock, an endoscopy (normal), preparation, and colonoscopy (normal) were performed. The patient was then submitted to computed tomography angiography, which identified a Meckel's diverticulum loop in the cecal area of the right iliac fossa with active bleeding inside the diverticulum. Laparoscopy was performed. In the intraoperative period, after identifying the Meckel's diverticulum, the wall of the small bowel loop was opened, and the optical device was passed through the opening to perform enteroscopy. An ulcer inside the Meckel's diverticulum with active bleeding was identified. A segmental enterectomy with lateral enteroanastomosis was performed.
