MULTIVISCERAL ROBOTIC RESECTION: LEFT HEPATECTOMY AND RECTOSIGMOIDECTOMY
This video presents a unique case of multivisceral robotic resection, involving a left hepatectomy and rectosigmoidectomy, performed using the da Vinci Xi robotic platform. The patient was diagnosed with a sigmoid tumor and a solitary metastasis in the left hepatic lobe. This innovative surgical strategy demonstrates the simultaneous approach to managing multifocal malignancies with advanced robotic technology. The hepatectomy began with the mobilization of the left hepatic lobe through the dissection of the triangular ligaments, providing adequate exposure to the liver segments. Parenchymal dissection targeted the second and third segments, using monopolar and bipolar forceps. The careful and controlled dissection minimized blood loss and preserved as much healthy liver tissue as possible. A laparoscopic stapler completed the hepatic parenchyma resection, ensuring thorough liberation of the surgical specimen. The rectosigmoidectomy involved the strategic mobilization of the lateral colon wall, with exposure and ligation of critical blood vessels. A linear stapler resected the lesion, effectively separating the surgical specimen from the remaining colon. After sectioning the colon, the specimen was carefully removed, and an anastomosis was performed using a circular stapler, ensuring a secure and functional reconnection of the colon. This case showcases the utilization of the da Vinci Xi robotic platform with a single docking technique, allowing for simultaneous resection of both hepatic and colorectal lesions. This approach offers several unique advantages: unparalleled precision and control, reduced operative time, preservation of healthy tissues, and a minimally invasive procedure that results in reduced postoperative pain, shorter hospital stays, and faster recovery times compared to traditional surgeries. The innovative surgical strategy integrates cutting-edge technology with expert techniques to effectively treat multifocal and complex tumors. Overall, this video serves as a valuable educational resource for surgeons and medical professionals, highlighting the potential and effectiveness of robotic-assisted surgery in managing complex oncological cases. It illustrates the future direction of surgical oncology, where precision, innovation, and minimally invasive techniques improve patient outcomes.
