    Craniopharyngiomas are opportune lesions for resection via the endonasal route if not arising purely within the third ventricle. If so, the endonasal approach has increased risk of hypothalamic injury. Here we present the case of a 64 year-old male with a tubero-infundibular craniopharyngioma with secondary extension into the third ventricle. This tumor was resected via an expanded endonasal route and we detail the technical nuances in approaching such lesions entering the third ventricle from inferiorly.
