    Nongerminomatous germ cell tumors represent 1 to 4% of pediatric intracranial neoplasms often affecting the sella/suprasellar region. In this video, we describe an endoscopic endonasal resection of a persistent sellar tumor in a 10-year-old female who had previously undergone six cycles of induction chemotherapy per Children's Oncology Group's phase 2 clinical trial guidelines (ACNS1123). Key surgical steps are highlighted, particularly sphenoid body drill out for conchal pneumatization pattern, tumor resection, and reconstruction with nasoseptal flap.
