A 53-year-old male patient with a history of penetrating ocular trauma presented with vision loss and complete ophthalmoplegia on the left side. Imaging revealed a foreign body extending from the left orbit into the sella turcica. The foreign body was successfully removed via a combined intra-orbital and endoscopic transnasal transsphenoidal approach. The dura over the pituitary gland was slightly damaged, but without CSF leak. The defect was successfully covered with spongostan and tisseel.
