Unlocking of the cavernous sinus and optic nerve decompression via an extradural anterior clinoidectomy, followed CN3 decompression at the cisternal segment and oculomotor triangle, and decompression of the cavernous sinus lateral wall and Meckel’s cave. NTR was achieved. Advantages of the minipretemporal approach to lesions of the cavernous sinus include minimal brain retraction for access to lesions extending to the lateral cavernous sinus wall, low risk to orbital structures, and minimal globe retraction.
