We present the case of a 52-year-old male with progressive gait disturbance and episodic uncontrolled tearing due to a large right petroclival meningioma. We performed a mini-combined transpetrosal approach (mini-CTPA) and described the procedure step by step. The mini-CTPA minimizes the surgical footprint of the combined transpetrosal approach (CTPA), from soft tissues to bony drilling, reducing approach-related morbidities focusing the attention on the petrous ridge while maintaining optimal corridors, surgical exposure, and maneuverability.
