Treatment of a Giant Fusiform Vertebrobasilar Aneurysm in a Hybrid OR: M2-RAG-P2 Bypass and Endovascular Coiling
Managing vertebrobasilar fusiform aneurysms requires flow reversal and aneurysm thrombosis while preserving critical vasculature. We present a giant fusiform vertebrobasilar aneurysm treated via M2-RAG-P2 bypass and endovascular coiling in a single session. In two-stage approaches, bypass occlusion while awaiting aneurysm takedown is a concern since the bypass competes with the higher flow basilar while the aneurysm is present. The hybrid OR enabled immediate aneurysm coiling, decreasing the time of competing flow and risk to bypass patency.
