We present the case of a 11-year-old female with sphenoid sinusitis and extradural empyema of the left middle cranial fossa. The patient presented complaining of progressive headaches and fever for 3 days. On examination, they had reduced sensation over left V1 and V2. The patient underwent an endoscopic transsphenoidal and transpterygoid approach to drain the collections, which was completed without complication. The patient made a complete recovery.
