Occipital Transtentorial Approach for Resection of Papillary Tumor of Pineal Region: Two-Dimensional Operative Video
Papillary tumors of the pineal region (PTPRs) are rare neuroepithelial neoplasms with <180 reported cases since first being described in 2003. We present a case of biopsy-proven WHO grade 2 PTPR in a 46-year-old male who presented with symptomatic obstructive hydrocephalus. A right occipital transtentorial approach was chosen to leverage gravity retraction, minimize manipulation of the dominant hemisphere, and maximize tumor exposure. We illustrate key anatomical landmarks to guide circumferential tumor dissection.
