A transpterygoid transcavernous and trans-quadrangular space exposure were initially performed. An inferior and medial transpetrosal approach was then performed posterior to the carotid anterior genu followed by a trans-jugular tubercle tumor removal. The tumor removal was continued posteromedially to clear the subarachnoid tumor within the cerebellopontine angle. The infiltrated anterolateral CS wall was widely opened to superficially debulk the tumor toward the superolateral compartment of the cavernous sinus. Reconstruction was performed with the pedicled nasoseptal flap.
