This video presents the case of a 52-year-old male with a clival chordoma and left sixth nerve palsy, showcasing the use of intraoperative ultrasound to enhance the transclival resection approach. The surgical anatomy, critical landmarks, and boundaries are highlighted to avoid neurovascular injury. The integration of ultrasound provides real-time imaging for tumor localization and margin delineation. This educational tool demonstrates the safety and efficacy of this minimally invasive technique in skull base surgery.
