    A 60-year-old male with months of dysphonia and an episode of breathing difficulty was diagnosed with a clival chordoma with extension dorsolateral to both atlanto-occipital joints. A multipronged tumor resection approach was planned. He underwent an occiput-C4 fusion, followed by endoscopic endonasal transclival resection. A few weeks later, subsequent left and right transcondylar approaches were performed on consecutive days. Postoperatively, he recovered well and has received proton beam therapy.
