A 60-year-old patient with a diagnosis of ameloblastoma of the right posterior maxillary sinus was referred to our care for further management. Total en bloc resection was achieved through a combined endoscopic and transoral pterygomaxillectomy. Reconstruction was achieved using a palatal island flap and a left composite chondromucosal nasoseptal flap.
