The use of digital applications in palliative medicine – A meta-analysis
Hintergrund Palliative medicine, characterized by a "high person – low technology" approach, emphasizes human connection. This prompts an exploration into the role of digital technologies within palliative care.
Methode We conducted a meta-analysis of prospective, randomized studies investigating the utilization of digital technologies in palliative care, comparing them with control groups. Digital technologies were defined as applications of data, algorithms, and/or algorithmic systems in healthcare. A systematic search of the MEDLINE/PubMed database and the Cochrane Library was conducted, following the Cochrane methodology.
Ergebnisse Out of 3157 potential studies identified, 806 full texts were reviewed, resulting in 25 studies meeting the inclusion criteria, with 19 studies included in the quantitative analysis. Symptom burden and control were examined in 14 studies (74%) involving a total of 2474 patients. The mean symptom burden for both the digital technology and control groups remained consistent at 0.32±0.22 on the normalized scale from study commencement to conclusion (with 0 representing no symptom burden and 1 representing maximum symptom burden). Six studies were included in the quantitative analysis, indicating no discernible advantage or disadvantage associated with the use of digital technologies in palliative care. Quality of life (QoL) was assessed in eight studies comprising 1521 patients. At baseline, the mean QoL for both groups was 0.67±0.08 and 0.68±0.09, respectively, on the normalized scale (ranging from 0 for poor QoL to 1 for perfect QoL). There was no significant difference observed between the experimental and control arms regarding quality of life at the study's end, nor in terms of Cohen's d, a measure of effect size, between the study's onset and conclusion. Acceptance of digital technologies, as defined, was evaluated in 7 out of 19 studies included in the quantitative analysis, indicating an overall acceptance rate of 80.2% across all seven studies.
Schlussfolgerung When used judiciously, digital technologies may be accepted within palliative care and may not impact symptom control or quality of life.
