Management of Benign Ovarian Cyst with Ultrasound-Guided Aspiration and Sclerotherapy

Purpose Background: Ovarian cysts represent fluid-filled sacs found within or on the surface of the ovaries, manifest a spectrum of clinical presentations. The management of ovarian cysts demands a tailored approach, considering factors such as cyst type, size, and the presence of symptoms. Our study on the management of benign ovarian cysts with ultrasound-guided aspiration and sclerotherapy aims to offer a locally relevant, minimally invasive alternative to traditional surgical methods.
Materials and Methods: A total of 23 patients were treated from April 2022 to April 2023 out of which 21 were offered both aspiration and sclerotherapy and in 2 patients only aspiration was performed. Cyst puncture was performed transabdominally using a 21-gauge Chiba needle with direct sonographic guidance utilizing a 4.5 MHz transducer. A single dose of Injection Methotrexate (30 mg) was then injected into the cyst. All procedures were conducted on an outpatient basis.
Results: Lower abdominal ultrasonography (USG) assessments were conducted at 1, 3, and 6 months for all patients. In instances of persistent cases, a second intervention involving cyst aspiration and methotrexate injection was performed. Out of 21 cases which were offered both aspiration and sclerotherapy 16 cases showed complete resolution after first session. Five patients showed partial response. These 5 patients were offered the second session of aspiration and sclerotherapy after which 3 of these showed complete resolution and in 2 cyst was persisting. The 2 patients who were offered aspiration only (as they were pregnant and methotrexate could not be administered) showed recurrence within 6 weeks with cysts reaching to almost of same size as previously. Second session of aspiration was performed and both again showed recurrence.
Conclusion: Aspiration and sclerotherapy is a reliable and effective alternative to surgery in treatment of simple ovarian cyst with a success rate of 90%. Sclerotherapy plays a vital role in this treatment and is far more superior than aspiration alone.
