Advanced training in systemic oncologic therapy in German Otorhinolaryngology departments – A survey of the DGHNO Oncology Working Group

Introduction Systemic oncologic therapy (SOT) is of high importance in head and neck oncology training. The requirements for the 12-month advanced training program for SOT are regulated by the state chambers of physicians. Our aim for this survey was to determine the presence of SOT training certifications and authorizations.
Material / Methods An online survey (Limesurvey) on SOT for head and neck cancer was conducted from 10/2022-07/2023. In the first part, qualifications, certifications, and treatment numbers with a focus on advanced training in SOT were covered. Additionally, palliative treatment standards and general assessments on cancer immunotherapy were queried. A total of 168 German Otorhinolaryngology departments were contacted.
Results 51 institutions completed the survey and were available for analysis (overall response rate: 30.4%; University Medical Center response rate: 70.0%). At least one person at 22 of participating institutions (n=51) completed SOT training (range: 1-5), among these 14 were from University Medical Centers. In 11/22 institutions advanced training was offered to fellows (no response: n=30). At least one person at 28 of the participating sites also carries an authorization for SOT training. Among these, 8 centers have the full 12 month training authorization, 4 institutions have a 6 month training authorization and 16 sites collaborate with another institution for SOT training.
Discussion A person with advanced training in SOT is available at 43.1% of participating sites. However, SOT is also carried out at institutions which have not completed this training.
