Transcavernous Resection of Adenoma Embedded in Medial Wall in MRI-Negative Cushing Disease

    A 26-year-old woman with a history of Cushing disease and biochemical evidence of pituitary source. Initial MRI is negative. Patient underwent inferior petrosal sinus sampling showing lateralization toward the right side. New MRI with a LAVA sequence showing signs of adenoma on the right side of the sella, entering the cavernous sinus wall. This video pinpoints the technique to access the medial wall of the cavernous sinus and the main medial wall resection criteria.
