    The treatment of Rathke's cleft cysts (RCC) requires marsupialization of the cystic contents. The ideal trajectory to access the cyst mitigates the risk of cerebrospinal fluid leak by tailoring the dural opening while preserving anterior gland. By clearly differentiating between normal gland and RCC, intraoperative ultrasound enables the surgeon to preserve glandular function and achieve disease resolution.
