An 84-year-old male presented with a left intraconal mass incidentally identified on imaging. Orbital decompression was performed on the left side. The periorbita was incised with a sickle knife exposing the orbital fat and the medially displaced medial rectus muscle. The medial rectus was incised longitudinally and bluntly separated. Blunt dissection was then used to release the tumor, and ultimately deliver it into the nasal cavity. The tumor was then released from its remaining attachments.
