Psychologische Suchttheorien als Erklärungsansätze

Zusammenfassung

Ziel: Die systematische Darstellung psychologischer Suchttheorien zur Erklärung charakteristischer Suchtphänomene. Methode: Eine Literaturrecherche in PUBMED ergänzt durch deutsche Literatur. Die Darstellung von Suchttheorien erfolgt in Bezug auf grundlegende Komponenten und Erklärungsprinzipien des Verhaltens für charakteristische Suchtphänomene. Ergebnisse: Notwendige Komponenten für die Darstellung von Suchttheorien sind ein Belohnungsund Kontrollsystem sowie ein Motivations- und Handlungssystem. Eine lerntheoretische kognitive Sichtweise erklärt die verstärkende Wirkung des Substanzkonsums mit der Entwicklung von impliziten und expliziten Erwartungen im Gesamtkontext mit natürlichen Verstärkern. Die positive Wirkung von Substanzen zur Steigerung von Geselligkeit, Freude und Lust oder Leistungsfähigkeit wird zunehmend ersetzt durch eine negative Verstärkung als Unterbrechung oder Abschwächung unangenehmer Situationen und Gefühle. Die Myopie-Hypothese erleichtert den Zugang zum Suchtverhalten. Ein Stresskonzept kann mittels negativer Verstärkung die Rolle von Stresssituationen für Rückfälle erklären. Bestimmte Persönlichkeitsfaktoren sowie traumatische Erfahrungen erschweren die Kontrolle über Kognitionen und Affekte und tragen zu Stresssituationen bei. Folgerungen: Nur eine Integration verschiedener Theorieansätze kann die Vielfalt von Suchtphänomene ausreichend erklären. Zwischen neurobiologischen und psychologischen Suchttheorien ergibt sich eine zunehmende Übereinstimmung hinsichtlich eines Belohnungs und Kontrollsystems. Die Funktion von Flow-Zuständen für die Sucht bedarf weiterer Klärung.

Summary

Aim: A systematic review of psychological theories of addiction to get explanations for typical addiction phenomena. Methods: A literature recherche in PUB ED about psychological theories of addiction and a supplement of German literature. In ordert to explain addiction phenomena, basic components of behavior organization and explaining principles of behavior are described by these theories. Results: Basic components of behavior organization are the reinforcement system, the control system and the motivation and acting system. Explanation principles are classical and operant conditioning, model learning and most important implicit and explicit expectations. The positive reinforcement of substance use for the enhancement of gregariousness, pleasure and achievement will be replaced by negative reinforcements of growing aversive consequences. Personality variables and traumatic experiences aggravate affective and cognitive control and activate stress mechanisms.The myopia-hypothesis facilitates understanding of addiction behavior. Consequences: Only an integration of different theories of addiction may explain the different trajectories of addiction. Neurobiological and psychological theories coincide as to the assumptions of an reinforcement and a control system. The role of flow status for addiction should be clarified.
