Zusammenfassung

Aufgrund der komplexen anatomischen und biomechanischen Eigenschaften desGewebesstellen Knorpelschäden den behandelnden Arzt trotz erheblicher Fortschritte in den letzten Jahrzehnten vor weiterhin nahezu unlösbare Proble-me. Die Behandlungsmöglichkeiten reichen von einer Hilfsmittelversorg ung und physikalischen Therapie über systemische undintraartikuläre Medikation bishin zueinerVielzahl von operativen Maßnahmen. In der letzten Gruppe muss man im Wesentlichen zwischen den reparativen und den rekonstruktiven Therapieansätzen unterscheiden. Für eine Wiederherstellung der originären hyalinen Gelenkfläche kommt nur die letzte Gruppe in Frage, wobei nur der Transfer eines körpereigenen Knorpel-Knochen-Zylinders oder die osteochondrale autologe Transplantation in der Lage ist, im Rahmen eines operativen Eingriffs einen sol-chen Knorpeldefekt zu rekonstruieren.

In dieser Übersicht werden die Möglichkeiten der heutigen Knorpeltherapie kritisch betrachtet. Trotz aller Diskussion zum Thema Knorpel dürfen eine bandstabile Gelenkfüh-rung, eine korrekte Gelenkachse und eine gute Muskelfüh-rung selbstverständlich nicht außer Acht gelassen werden.

Summary

Even with the great variety of treatment modalities on offer today, the healing of cartilage injuries remains problematic. Due to its complex structure, it has proven to be very difficult to repair or reconstruct an injured hyaline cartilage joint surface. The treatment options range from orthotic devices and physical therapy to systemic and intraarticular medication, as well as to a variety of surgical interventions. Among the surgical treatment forms one must differentiate between the repair and the reconstruction of hyaline joint surfaces. In the latter group only the osteochondral autologous transplantation or OAT-procedures allows for the reconstruction of a cartilaginous lesion with hyaline cartilage as part of a single procedure.

This paper will attempt an overview of most common therapeutic approaches to cartilage injuries available today. Even with the ongoing discussions with regard to hyaline cartilage, the basics such as the ligamentous stability of the affected joint, the mechanical axis of the extremity and good neuromuscular control must not be allowed to slip out of our view.
